Payment Options: 

There are four types of payment options available to pay human subject incentives; CASH, CHECK, COUPON, and VISA CARD.  The type of payment you use for a study must be authorized by the IRB unless your study is exemptThe Principal Investigator is responsible for knowing which incentives types may be provided to subjects.  The payment option grid below outlines how each incentive may be distributed, delivery options, and the documentation required.

Documentation Receipts: 

In order to provide evidence of distribution, receipts are created upon making your request for funds in the HSIP system.  The information required on the receipt is determined by the study tier level.  Tier levels are explained in the HSIP request form and are assigned to a study by the IRB based on payment amount and study risk.  The HSIP system will produce receipts when you make your request.  If you need receipts before you make your request, blank HSIP receipts are available below:

If not all funds are used, there are methods to return the excess incentives to the HSIP Office.

Returning Excess Funds

To report subject information to the HSIP Office by spreadsheet for checks or studies that don’t require a signed receipt, there are templates below that can either be sent by email or attached in the request form by the Unit Administrator.

Study Coordinator Toolbox:

These tools are optional and provided as tools for the study coordinator in tracking incentives for a study!

Tools are meant to be used as an aid for internal tracking.  It is not necessary to submit these sheets to HSIP or use them at all. These tools may be modified to better suit the needs of the study.  Questions or tool requests may be made through subject-incentives@umich.edu.
